3. Line 52 of the 2009 IRS Form 5695 is used to claim the tax credit. Download IRS Form 5695 for tax year 2009. Instructions on using the document are contained withing the form itself.

4. Refer to your invoice to complete the worksheet and IRS form, and bring all documents to your tax professional.
